Job Details

• Employment Type: Travel Contract (Home Health / Homecare nursing)
• Location: Mission Viejo, CA (Orange County) – territory includes Newport Beach, Costa Mesa, Tustin, Irvine, Lake Forest
• Weekly Estimated Pay: $2,190 - $2,550
• Schedule: Thursday–Monday or Friday–Tuesday, 8:00 AM – 5:00 PM
• Shifts per Week: 5 (8-hour days)
• Guaranteed Hours: 40 per week
• Contract Length: 13 weeks
• On Call: None
• Productivity: 27.5 - 30 points per week
• EMR: HCHB (Homecare Homebase) required
• Start Date: April 27, 2026

Job Requirements

• Active Registered Nurse (RN) license in the state of California
• Minimum 1 year of home health or homecare nursing experience with a Medicare-certified facility
• Proficiency with HCHB (Homecare Homebase) EMR
• Strong organizational, clinical documentation, and communication skills
• Valid driver’s license and reliable transportation for field nursing visits

Responsibilities

• Provide skilled nursing care to patients in their homes across assigned Orange County territory
• Develop, implement, and evaluate individualized patient care plans as a Home Health Nurse
• Coordinate with interdisciplinary team members, including case managers and therapists, to ensure optimal patient outcomes
• Document all patient interactions and skilled nursing care using HCHB EMR accurately and timely
• Educate patients and families on disease processes, medications, wound care, and home care techniques
• Maintain productivity standards of 27.5 - 30 points per week while delivering high-quality, patient-centered care
