Job Requirements
- Active RN license in North Carolina or compact state licensure
- Minimum 1 year of recent nursing experience (home health/homecare experience preferred)
- Current BLS certification (American Heart Association preferred)
- Strong patient assessment, clinical decision-making, and documentation skills
- Excellent communication, time management, and ability to work independently while managing a caseload
Responsibilities
- Provide skilled nursing care to patients in their homes with a focus on safety, quality, and compassionate service
- Perform comprehensive patient assessments, develop and implement individualized care plans, and monitor patient progress
- Educate patients and families on diagnoses, medications, wound care, and self-care techniques to support positive outcomes
- Coordinate care with interdisciplinary teams and community resources to ensure continuity of care
- Accurately document patient visits, interventions, and outcomes in the electronic medical record in a timely manner
