- Weekly Estimated Pay: $2,190 – $2,540
- Job Title: Registered Nurse (RN) / Home Health Nurse / Homecare RN
- Location: Trumbull & Stamford, CT — Fairfield County, Connecticut (local candidates preferred)
- Setting: Homecare / Home Health Agency
- Start Date: June 1, 2026
- Contract Duration: 13 weeks
- Hours per Week: 40 (5 shifts/week)
- Shift: 8:00 AM – 4:30 PM, Monday to Friday
- On Call/Weekend: None
- Dress Code: Any color scrubs
- Mileage: Mileage reimbursed between second patient and last patient visits
Job Requirements / Qualifications
- Current RN license in the state of Connecticut (required)
- Minimum 1 year of RN experience; prior homecare / home health nursing experience preferred
- Proficient with HCHB (Homecare Homebase) and OASIS documentation
- Strong wound care, skilled nursing, and case management abilities
- Ability to work independently, manage a homecare caseload, and prioritize visits
- Reliable transportation and valid driver’s license for travel between patient homes
- Excellent communication and coordination skills with interdisciplinary care teams
Primary Responsibilities
- Provide skilled nursing care (home health) to patients in their homes, maintaining high clinical standards
- Conduct comprehensive RN evaluations, admissions, and recertifications following home health protocols
- Deliver wound care, medication management, and case management services
- Document patient care accurately using HCHB and OASIS to meet regulatory and agency requirements
- Coordinate care with interdisciplinary team members, families, and community resources
- Cluster patient visits for route efficiency across Trumbull and Stamford to optimize homecare delivery
