Job Requirements / Qualifications
- Active Registered Nurse (RN) license in Georgia or multistate compact RN license
- Minimum 1 year nursing experience; hospice or palliative care experience preferred
- BLS certification (American Heart Association or equivalent preferred)
- Strong clinical assessment, symptom management, and documentation skills
- Excellent communication, interpersonal skills, and ability to work both independently and within a multidisciplinary team
- Compassionate, patient-centered approach to end-of-life care and family support
- Palliative care certifications (e.g., CHPN) or hospice training are a plus
Responsibilities
- Deliver skilled hospice nursing care, including symptom management and medication administration per individualized care plans
- Provide palliative and end-of-life care focused on patient comfort, dignity, and family-centered support
- Collaborate with physicians, social workers, chaplains, and other healthcare professionals to coordinate comprehensive hospice care
- Educate patients and families about disease progression, symptom control options, and goals-of-care conversations
- Monitor, assess, and accurately document patient status, interventions, and responses in the medical record to meet hospice documentation standards
- Offer emotional support and guidance to patients and loved ones while maintaining professional boundaries and empathy
- Participate in interdisciplinary team meetings and contribute to care planning and quality improvement initiatives
