Job Requirements
- Active Alabama LCSW license required
- At least 1 year of recent acute care social work or inpatient hospital case management experience required
- Experience with psychosocial assessments, discharge planning, and community resource coordination
- Ability to work effectively in a large academic medical center or hospital environment
- Flexibility to support different inpatient units as needed
- Strong communication, teamwork, and patient-first approach
- Experience with Cerner EMR is preferred
Responsibilities
- Provide inpatient social work services for patients and families across assigned units
- Complete psychosocial assessments and identify patient support needs
- Develop care plans in collaboration with the interdisciplinary care team
- Coordinate discharge planning and connect patients with appropriate community resources
- Participate in interdisciplinary rounds and ongoing case management coordination
- Maintain accurate and timely documentation in the electronic medical record
- Float between units as needed to support patient care and operational needs
