We are seeking a Travel Medical Social Worker / Case Manager experienced with EPIC EMR to support acute care, ICU, mother-baby, labor & delivery, and ED patients. Minimum Qualifications – Social Work / Case Management

  • MSW required; LICSW or LSWAIC preferred (LSWAIC acceptable if supervision by MHS is not required)
  • Minimum 2 years acute care hospital experience OR 1 year acute care + 1 year Hospice, Home Health, or geriatric care
  • Experience documenting and coordinating care in EPIC EMR (required)
  • Proven skills in substance use screening (SBIRT), mental health evaluations (SAFE-T), and capacity evaluations
  • Experience with behavior plans, behavioral health referrals, tele-psych evaluations, discharge planning, and difficult placements
  • Knowledge of Single Bed Certs / Involuntary Treatment Act (ITA) processes and end-of-life conversations
  • Prior exposure to home health, hospice, SNFs, Medicaid/Medicare programs, and insurance authorizations strongly preferred
  • First-time travel clinicians are welcome to apply

Primary Responsibilities

  • Perform mental health and capacity evaluations for acute care and emergency patients
  • Develop and implement behavior plans and coordinate complex discharges
  • Facilitate tele-psych evaluations and process behavioral health referrals
  • Coordinate end-of-life conversations and support families through difficult placements
  • Manage Single Bed Certs / ITA processes and related documentation
  • Provide social work services across medical-surgical units, ICU, mother-baby, labor & delivery, and ER as assigned
  • Float to Valley location as needed and collaborate with multidisciplinary teams for care coordination
  • Document all assessments, care plans, and interventions in EPIC EMR
