- Healthcare Setting: On-site hospital / clinical environment

- Assignment Length: 13-week travel assignment
- Schedule: Monday-Friday, 8:00 AM-4:30 PM
- Shift Duration: 8-hour days
- Hours: 40 hours per week, including a 30-minute lunch
- Weekend Requirement: No weekends
Licensed Master Social Worker Job Requirements
- Active Licensed Master Social Worker (LMSW) license required
- Healthcare social work or discharge planning experience preferred
- Strong communication, documentation, and care coordination skills
- Ability to work effectively with interdisciplinary healthcare teams
- Knowledge of transitions of care and patient support services
Social Work Responsibilities
- Coordinate and support discharge planning for patients in a healthcare setting
- Collaborate with clinical staff, case management, and interdisciplinary teams to ensure safe transitions of care
- Assess patient needs related to discharge planning, community resources, and support services
- Document interventions, care plans, and patient progress accurately and thoroughly
- Provide resource coordination and patient advocacy throughout the care continuum
- Support evolving departmental needs as responsibilities change over time; psychosocial duties are not currently part of the role but may be added in the future
