Job Requirements / Qualifications
- Active Licensed Clinical Social Worker (LCSW) credential required (California LCSW preferred)
- Minimum 1 year recent experience in a leadership or management role in social work or case management
- Recent experience with California Dignity Health systems highly preferred
- Proven skills in leadership, communication, organization, and staff development
- Experience overseeing multidisciplinary teams (social workers, RNCCs, CCAs, UM analysts)
- Familiarity with utilization management (UM), case coordination, discharge planning, and regulatory compliance
Responsibilities
- Manage and provide operational oversight for a team of 11 social workers, 8 RNCCs, 3 CCAs, and 1 UM analyst (23 direct reports total)
- Coordinate daily social work and case management operations to ensure high-quality patient care and efficient workflows
- Lead case coordination, utilization management, discharge planning, and care transitions in collaboration with clinical teams
- Support, mentor, and evaluate staff—promote professional growth, training, and performance improvement
- Work closely with RN leadership and report to department leadership to align goals and metrics
- Maintain compliance with hospital policies, California regulations, and accreditation standards
- Promote a positive, efficient, and patient-centered work environment focused on care coordination and quality outcomes
