Job Requirements
- Minimum 3 years of clinical experience as a Home Health RN or equivalent home nursing experience
- Proven experience with OASIS assessments and HCHB (Homecare Homebase) charting
- Current Registered Nurse (RN) licensure in Tennessee (or active compact state RN license as applicable)
- Current CPR/BLS certification (AHA preferred)
- Valid driver’s license, current vehicle insurance, and reliable transportation for home visits
- Strong documentation and clinical documentation skills, home health case management, and patient education experience
Responsibilities
- Deliver skilled nursing care to patients in their homes, ensuring patient safety and adherence to best practices in home health nursing
- Perform comprehensive patient assessments and complete accurate, timely OASIS documentation
- Use HCHB (Homecare Homebase) for all clinical charting and electronic documentation
- Develop, implement, and update individualized care plans in collaboration with the interdisciplinary team and physicians
- Provide patient and family education on disease management, medication administration, wound care, and home safety
- Monitor patient progress, recognize changes in condition, escalate concerns, and coordinate follow-up care
- Support care coordination and case management activities to optimize outcomes for home health patients
