• Setting: Home Health Nursing
• Start Date: November 10, 2025
• Contract Duration: 13 weeks
• Shifts per Week: 3 (blocked schedule: Friday, Saturday, Sunday; 12-hour day shifts, 36 hours per week)
• Estimated Weekly Pay: $1,935 - $2,150
• Employment Type: Travel Nursing Contract (On-Site)

Registered Nurse Qualifications

• Minimum 12 months of recent Home Health RN experience (within the last 3 years)
• Proficiency with HCHB (Homecare Homebase) – required
• OASIS documentation experience – required
• Active Wisconsin or Compact Registered Nurse License – required (must be in hand at time of submission)
• Current BLS certification – required

Home Health RN Responsibilities

• Provide skilled nursing care to patients in their homes across the Greater Milwaukee area
• Complete three patient admissions per day and approximately two additional home visits daily
• Utilize HCHB and OASIS for accurate, timely clinical documentation
• Collaborate with interdisciplinary healthcare teams to develop and implement individualized care plans
• Educate patients and families on disease management, medications, and self-care techniques
• Ensure compliance with all regulatory, clinical, and safety standards
